There are three ways to make a claim for dental injuries and emergencies

You can claim online, by post, or by email. If you've had emergency dental treatment in the UK, log in to our online portal below to claim.

To claim by post, click below to download the relevant form. Or use the link below to submit your claim via email.

If you've had emergency dental treatment in the UK, you can make a claim online. You'll need to have the following to hand:

Your details, including your membership number
Details of the dentist who treated you
Details of the appointment and the treatment you received
A scan or picture of the receipt from the treatment, in one of these file formats: BMP, JPEG/JPG, ODT, PDF, PNG or TIF

To claim by post, first download the correct claim form from the list below.

Fill in the details and return it to Denplan with proof of the dental treatment you've had.

Claiming via email is much like claiming by post, to get started you'll need to download the correct claim form from the list below.

Once you have the form, fill in the details and return it to [email protected] with proof of the dental treatment you've had.
